What are Perimenopause and Menopause?
Perimenopause and menopause are key phases of hormonal transition related to aging that bring about distinct changes in your body. Perimenopause often begins in your 40s, while menopause typically occurs between ages 45 and 55.
Understanding Perimenopause vs. Menopause
- Perimenopause is the transition leading up to menopause. It involves fluctuating hormone levels, often resulting in unpredictable symptoms. This stage can last several years and is characterized by irregular periods, hot flashes, mood swings, and changes in sexual health.
- Menopause is defined as the point when you have not had a menstrual period for 12 consecutive months. It marks the end of reproductive cycles and is associated with symptoms such as hot flashes, night sweats, irritability and mood changes, sleep problems, vaginal dryness and cognitive effects like difficulties concentrating and "brain fog."
Symptoms You May Experience
Many symptoms overlap between perimenopause and menopause, but they may present differently:
- Irregular Periods: During perimenopause, you might notice changes in cycle length, flow, or frequency, whereas menopause means menstrual periods have ceased entirely.
- Hot Flashes and Night Sweats: Both stages involve hot flashes, but during perimenopause, they tend to be more unpredictable.
- Mood Swings: Fluctuating hormone levels during perimenopause can lead to increased irritability or anxiety, while menopause may bring longer-lasting mood changes.
- Cognitive Effects: Both phases can bring on a feeling of "brain fog" or difficulties with memory and concentration.
- Sleep Disruptions: Fluctuating hormone levels, especially during perimenopause, can lead to sleep difficulties.
- Changes in Sexual Health: Both stages can result in vaginal dryness, decreased libido, or discomfort during sex, which can be especially challenging without inclusive and affirming care.
- Physical Changes: Unexplained weight gain, especially around the abdomen, and changes in skin elasticity or hair growth.
How FOLX Clinicians Can Help Manage Perimenopause and Menopause
FOLX clinicians take a personalized approach to your care, recognizing that both perimenopause and menopause require thoughtful, tailored support. We also understand the nuances of queer and gender diverse experiences with menopause and are committed to providing care that respects your identity. Whether you're experiencing natural menopause or menopause as a result of hormone therapy or surgery, we ensure that your care plan is comprehensive and affirming.
Treatment options include:
- Hormone Therapy: We offer hormone replacement therapy (HRT) to alleviate symptoms such as hot flashes and mood changes. This includes options like combined oral contraceptives, estradiol, progesterone, and local vaginal estrogen during perimenopause, and HRT for menopause-related symptoms. Testosterone therapy can also be used for symptoms of low-libido during perimenopause. And some people use menopause as a time to explore a broader gender transition for themselves. Your FOLX clinician can discuss your goals and treatment options to provide a well-rounded plan.
- Non-Hormonal Treatments: If hormone therapy isn’t right for you, we offer non-hormonal medications to manage symptoms such as hot flashes or vaginal dryness. There are multiple medication options to help treat menopause symptoms if hormone therapy is not right for you. Your FOLX clinician can discuss all of your options to help you choose the right treatment for you.
- Lifestyle and Nutritional Guidance: Nutrition, exercise, sleep, and stress management can significantly improve quality of life during both perimenopause and menopause. Our clinicians work with you to create a comprehensive lifestyle plan that fits your needs.
- Mental Health Support: Whether it's perimenopause or menopause, mood changes can be tough. FOLX offers treatment options for anxiety, depression, and insomnia, along with therapy and referrals to LGBTQIA+-inclusive mental health professionals.
- Community Resources: As a FOLX member, you gain access to our Community Platform, where you can connect with others experiencing similar transitions, share insights, and find support.
By integrating care that acknowledges the unique aspects of perimenopause, FOLX ensures you receive the comprehensive, affirming support you need during this crucial life stage. Not only cis-women experience perimenopause and menopause symptoms and we are here for you!
How to Prepare for Your Perimenopause or Menopause Visit with FOLX
To get the most out of your virtual visit, consider tracking your symptoms in advance. Pay attention to patterns in hot flashes, night sweats, mood shifts, or any changes in your sexual health. If you're already using hormone therapy, keeping a journal of how you're feeling can help your clinician make necessary adjustments to your dosage or medications.
Additionally, have a list of your current medications and any supplements you’re taking. It is also helpful to list the treatments you have already tried to help your symptoms. This helps your FOLX clinician provide the most informed and holistic care possible.
FOLX's Unique Approach to Perimenopause and Menopause Care

This care may also include ordering recommended screenings and lab tests recommended during this stage of life. This may include mammograms (regular mammograms are required to receive MHT at FOLX), metabolic screening, dexa scans, thyroid screening and other primary care needs.
Beyond clinical care, you also gain access to a supportive community. Our Community Platform connects you with others going through menopause, offering a space to share stories, resources, and encouragement.
Frequently Asked Questions
Can FOLX clinicians prescribe hormone therapy for menopause?
Yes, FOLX clinicians can prescribe hormone replacement therapy (HRT) to help alleviate perimenopause and menopause symptoms.
What if I’m already on hormone therapy?
FOLX clinicians can adjust your existing hormone therapy to better suit your needs during menopause. We're here to ensure that your treatment remains effective and supportive of your overall well-being.
Is there a community aspect to menopause care at FOLX?
Absolutely! FOLX offers a Community Platform where you can connect with other members, share experiences, and access additional resources tailored to your journey through menopause.
Are hormone labs required or recommended during menopause?
No. Hormone labs are not required or recommended for the treatment of menopause. Professional organizations recommend the treatment of menopause based on the symptoms, as the hormone levels fluctuate greatly throughout the month and low levels are not required to treat symptoms.
Does estrogen cause cancer?
Menopausal hormone therapy does slightly increase the risk of breast cancer. This is why FOLX requires regular mammograms for breast cancer screening. There are also non-hormonal medications to help treat menopause symptoms for those you cannot have hormone therapy due to personal risk for breast cancer.
What types of hormone therapy do you offer at Folx?
FOLX can prescribe menopausal estrogen hormone therapy in the form of a patch, cream, vaginal cream, or oral tablet. Progesterone is prescribed orally or vaginally. Testosterone can be in the form of injections, gels, or cream
